When you're given the choice between confronting Otama or Wakasa, you should confront Wakasa. She's the target you are there to kill. Having done so, the two of you will exit into a small room, where Wakasa will give a speech. In order to assassinate her, interact with the gun on the back wall and she'll be killed in the cutscene that follows.
If you instead choose to confront Otama when given the choice after the tea ceremony, you'll chase her down and assassinate her, but a note on her corpse will reveal Wakasa to be the real target. You'll then have to go to her castle and kill her there instead.
